REQUEST FORM FOR ARCHIVED TEST REPORT INFORMATION

ITL strives to meet our customers needs at a minimum cost to them. To save time, money, and to cut back unnecessary paper, we have implemented the following policy.

ITL will keep a listing of all requests for all archived files e-mailed or faxed to you. At the end of the year we will bill your company at the following rate.

First 2 e-mails or faxes of archived reports per year will be at no charge, as long as your company has done business with ITL in the previous year. After the first 2, you will be charged $15.00 for the third e-mail or fax, each additional e-mail or fax after will be $5.00 each. Each mailed IES file on disk or hard copy of report will be $10.00 each. PDF files e-mailed will be charged at $10.00 each. Five or more ordered together will be discounted, call for quote.
